Histologic Graft-Versus-Host Disease Grade Analyzed

Am J Hematol; ePub 2017 May 26; Narkhede, et al

Histologic graft-versus-host disease (GVHD) grade correlated with clinical grade in a study involving 231 individuals. This suggests its potential role in predicting severity and treatment response of acute GVHD.

Participants underwent allogeneic HCT between 2005 and 2013. Investigators looked at results of 300 skin and GI tract biopsies. Among the results:

  • Histologic and clinical grade were significantly linked with day 28-treatment response.
  • Response was better in patients whose histologic grade was lower than its associated clinical grade.
  • A histologic grade higher than its associated clinical grade did not correlate with response.
  • Patients with both clinical and histologic GVHD grade had higher odds of non-relapse mortality (47% and 67%, respectively).
  • The same was true for all-cause mortality (57% and 29%, respectively).
