Survey Reveals Gaps in T1D Care Transition

Shortfalls seen in handoff from youth to adult care

There is a need for better information transfer between pediatric and adult providers as individuals with type 1 diabetes transition to adult care, according to a survey of 536 endocrinologists. Additionally, patients need improved mental health referral access.

The survey, which had a 13% response rate, found that:

• Only 36% said they often or always review pediatric records, and just 11% acknowledged receiving summaries for transitioning young adults with type 1 diabetes.

• More than 70% believe these activities are important for patient care.

• More than 9 in 10 said diabetes educators and dieticians were easily accessible, but only 4 in 10 said they had easy access to mental health professionals. Access was particularly challenging in nonacademic settings.
