Program to Improve Prescribing of Primarily Renally Cleared Oral Medications in Older Veteran Community Living Center Patients

Joseph Hanlon, PharmD, MS; Steven M. Handler, MD, PhD; and Sherrie L. Aspinall, PharmD, MSc

Dr. Hanlon is a health scientist with the Geriatric Research, Education and Clinical Center (GRECC), VA Pittsburgh Healthcare System (VAPHS) in Pennsylvania and a professor in the Department of Medicine (Division of Geriatric Medicine), the Department of Epidemiology, and the Department of Pharmacy and Therapeutics at the University of Pittsburgh (UPitt) in Pennsylvania. Dr. Handler is a health scientist with the VAPHS GRECC and is an assistant professor in the Department of Medicine (Division of Geriatric Medicine) and the Department of Biomedical Informatics at UPitt. Dr. Aspinall is a health scientist with the VA Center for Medication Safety in Hines, Illinois, and is an associate professor in the Department of Pharmacy and Therapeutics at UPitt. Drs. Hanlon, Handler, and Aspinall also are all health scientists at the VAPHS Center for Health Equity Research and Promotion.
