The surgeons thought someone else was going to stop the LMWH. “Interestingly, the hospitalist in the case made a notation in the chart after the neurological injury occurred, but dated the entry as before the fact. Doctoring the medical record never plays well to the jury—it is a clear sign of guilt!”
